Create a new document, 400px Width, 500px Height, 72dpi Resolution, white background
Press CTRL + V to paste from the clipboard
Press CTRL + T to free transform
Click the Maintain Aspect Ratio chain
Enter 50% in the Set Horizontal Scale field
Enter X value to 85px
Enter Y value to 135px
Press Enter twice
Select the Single Column Marqeue Tool from the toolbar
Press F8 to view Info tab
Press CTRL + K to open preferences
Press CTRL + 5 to goto Units and Rulers
Click and drag the Single Column Marqeue Tool until the X value is 81px
Release the mouse button
Press CTRL + C to copy to clipboard
Press CTRL + Shift + N to create a new layer, Press Enter
Press CTRL + V to paste from the clipboard
The new layer should be selected
Press CTRL + T to free transform
Enter 45000% in the Set Horizontal Scale field
Enter X value 305px
Enter Y value 136px
Press Enter twice
Move Layer 2 below Layer 1
Select the Magic Eraser Tool
Select Layer 1
Click the right side of the picture on the layer to remove the white space
Select Layer 2
Expand the window's width to give you room to work on the warp
Press CTRL + T to free transform
Click Edit in the menu bar, Click Transform, Click Warp
Move bottom right handle labeled 1 to the spot labeled 2. The Info window should show 393 for the W (Width) value.
Move the next five handles as shown in labels 2 - 6. Sorry, you just have to wing it instead of using values. If it looks a little inaccurate, it's because I had to redo it each time to do a screenshot.
